What should I bring with me?

Please bring the following to your first visit:

• Completed Paperwork ( Pt Information and History Forms)
  • Forms
  • A copy of your list of medications
  • A list of your Physicians / Health Care Practioners and their contact info.
• Insurance Card(s)
• Picture ID
• Referral / Prescription
  • Not always required – please call if you have any questions
• A desire to make “A Step Ahead”
